Review and close a risk
Set review dates, record decisions, reopen a risk, or delete a record.
Open the Risk Register, choose a risk, and open its full record. Lifecycle changes require Contributor, Admin, or Owner access to its project. Check the current status and review information before choosing an action.
Set review cadence and target resolution
Choose Lifecycle. Set Review cadence and Target resolution, then choose Save lifecycle. Target resolution is the date for bringing the risk within appetite; clear the date to remove that target. Save lifecycle is enabled when you have changed a value.
The record shows its effective next review date and whether the review is overdue. When you have completed a review, record it using Mark reviewed rather than only editing a date.
Record a decision
Open Actions and choose the decision that matches your review. Read the confirmation before applying it.
| Action | What it records or changes |
|---|---|
| Mark reviewed | Records today's review and resets the review clock using the review cadence. You can add an optional review note. |
| Accept risk | Records a formal decision to live with the risk as it stands. A written reason is required. The risk stays in the register with an acceptance date; treatment is no longer expected until the acceptance is reviewed again. |
| Dismiss risk | Takes the risk off active tracking without treating it. A written reason is required. It stops counting toward open totals and drops off review reminders, while its record and history remain. |
| Reopen risk | Returns a resolved or closed risk to active tracking, open totals, and the review cadence. |
| Change status | Moves the risk to another available lifecycle status and updates the register's reporting. Choose the new status and confirm. |
Status choices include Open, In review, Treatment planned, Treatment in progress, Mitigated, Transferred, Avoided, and Closed, subject to the current state. Acceptance and dismissal use their dedicated actions because they require a reason.
An unavailable action includes an explanation. For example, reopen a closed risk before changing its status or recording a new acceptance. An active risk is already being tracked, so it does not need reopening. Closed risks are no longer on a review cadence.
After saving, check the updated status, dates, and success message. Use Activity to review the recorded decision. If an action fails, the confirmation shows an error; correct the issue and retry, or cancel to leave the decision unapplied.
Delete a risk
Use Delete only when the record itself should be removed. Read Delete this risk?, then confirm Delete risk. Deletion removes the risk and its related activity from the register and cannot be undone. The full page returns you to the register after successful deletion.
Use Dismiss risk when you need to keep the record and history while stopping active tracking. If deletion fails, read the error message and check the register before retrying.
