Create a risk
Place a risk in its project and artifact context, then describe, score, and save it.
You need Contributor, Admin, or Owner access to the project where the risk belongs. The workspace needs an active Platform license; available artifact choices also reflect your access and the workspace's features.
Choose the context
- Open Risks and choose New risk.
- In Scope, select Project, then Artifact. Search the choices and use their pagination controls if needed.
- Complete the context for that artifact:
- For an assessment, select both Assessment item and Policy template.
- For an organizational document, select Document section.
- For a contract, select Contract section when a specific section applies, or leave No linked section.
- For a timeline, select Timeline event when applicable, or leave No linked event.
Choose the project and artifact first. The more specific choices depend on them; changing the broader scope may require you to select the context again.
Describe and save the risk
In Narrative, enter a clear Title. Use Description for what could go wrong and why it matters, and Scenario for the trigger or business situation. Add the primary and secondary impacts, category, and treatment or response details you know. Contract-related fields can capture the original clause and remediation notes.
In Scoring, record the factors you can support before and after controls. Leave factors unscored when you do not yet have enough information. In Lifecycle, choose the response and cadence and add target or next-review dates where appropriate.
Choose Save. Once the risk is saved, open it and assign accountable owners and reviewers from the eligible project members. See Score and treat a risk for the next steps.
If saving is blocked
Read the field messages and the form's error summary. A project and artifact are required. Assessment risks also require an assessment item and policy template; organizational-document risks require a document section. A title is required even when the remaining narrative is still being drafted.
If no project is available, confirm the workspace and ask a project admin to check your contribution access. If a context list cannot load, use its retry control before selecting a different item. Do not choose an unrelated context to bypass an error.
Closing a form with unsaved edits can show a discard confirmation. Keep editing to preserve the draft, or choose Discard changes when you want to abandon it.
